Home Within a Home
Beaumont strives to provide every Resident with their best life.
Home Within a Home is SALMON Health and Retirement’s approach to “culture change,” a revolution that has taken place in the way nursing homes operate.
“’Culture change’ is the common name given to the national movement for the transformation of older adult services, based on person-directed values and practices where the voices of elders and those working with them are considered and respected. Core person-directed values are choice, dignity, respect, self-determination and purposeful living.” – Pioneer Network
With Home Within a Home, we’ve redefined all aspects of our Beaumont environments – from management to health care to dining services – to create the flexibility for serving each long-term care Resident’s stated needs.
The guiding principle of Home Within a Home is that each individual Resident has access to all of the freedoms and rights that a traditional home offers. We believe it is our responsibility to create an environment where our Residents enjoy the freedom of self-expression, emotional and social support so they can live and function as unique individuals. In culture change nursing homes, the entire staff supports Resident choice, and everything else becomes secondary.
At Beaumont, all disciplines from nursing care to rehabilitation, housekeeping, recreation and dining services work together to accommodate Resident choice and provide them the most home-like environment possible.
Consistent staff scheduling ensures Residents, their families and staff come to know each other well and can develop close relationships. With this arrangement, our team is also able to more easily anticipate Resident needs, and Residents can relax and gain confidence in their caregivers, as can their families.
Through Home Within a Home, our Residents gain greater control of their daily lives and relationships change for the better on all levels.
All of our Beaumont locations participate in INTERACT (Interventions to Reduce Acute Care Transfers), a quality improvement program designed to improve the identification, evaluation, and communication about changes in a Resident’s condition. The overall goal of the INTERACT program is to reduce sending long-term care Residents to hospitals.
Hospitalization can be emotionally and physically difficult for Residents, and can result in complications, which are costly. Beaumont has been participating with the program since 2008 and was the leader in reducing re-hospitalization in its first year.
Our Home Within a Home initiative works hand-in-hand with INTERACT, because consistent staffing, identification and communication of condition changes and nutritional management all help us anticipate minor changes so we can prevent episodes of acute illness in our Residents.
