Admissions Process

Just a few steps to see if our program suits your needs.

  1. Visit the Center for a tour
  2. Fill out an application
  3. Have the client’s primary care physician fill out a medical report
  4. Submit insurance verification if necessary
  5. Optional visit by the Center director or social worker at the client’s home to become more familiar with the client’s interests and needs
  6. Begin attending the Center!

Payment Sources

We accept private payment, Tri-Valley Elder Services, MassHealth and Long-Term Care Insurance*

Private Long-Term Care Insurance policies sometimes include Adult Day Health services. Please refer to your policy for details regarding Adult Day Health coverage.

Tri-Valley Elder Services and Elder Services of Worcester can help defer the cost for those who are not MassHealth/Medicaid eligible. For more information, please call Tri-Valley Elder Services at 800-286-6640 or Elder Services of Worcester at 508-756-1545.

MassHealth/Medicaid will cover 100% of the cost of Adult Day Health services, after the individual has been screened by Tri-Valley Elder Services.

Navicare will cover 100% of the cost of Adult Day Health services, after the individual has been screened by Tri-Valley Elder Services.

Summit Eldercare Current Summit Eldercare participants whose care plan (developed by Summit's interdisciplinary team) includes Day Health can choose to attend Whitney Place Adult Day Health at Northbridge, instead of a Summit location. Summit pays for 100% of both care and transportation. For those who would like to learn about Summit or sign up for Adult Day Health can contact the Director at (508) 234-6481.

*Coverage begins after a probationary period as required by your insurance carrier. However, clients must pay privately and accept reimbursement from that carrier afterward.
